c． Mai ling address：常分為Current和 Permanent兩種。如二者相同，則另一個可填Same。要注意目前地址 （Current address）一定要填安全可靠的地址，以便你能及時地收到郵件，保證申請過程的順利進行。Address一般會分幾個欄目：如street，city state country。其中State是指美國的州，國際學生不要求填寫。
d.Telephone number：北京地區的電話可以用+86-10-xxxxxxxx表示，這裏86是中國區號，10是北京的長途區號010去掉0。其他地區電話可參照此法填寫。
e． E—mail address：這是目前最普遍使用的聯係方式。在某種意義上說是必不可少的。一個安全可靠的E—mail地址將給你的申請及聯係過程帶來極大的方便。填表時，E—mail地址要注意書寫清楚，拚寫無誤，以保證及時收到學校發來的所有信息。
f.Citizenship:中填P．R．China即可。
g. Date of birthday：一般按月／日／年的順序填寫。
(2)正式成績單(Official transcript)
①學校的要求。
首先我們來看學校對成績單的要求：

正式成績單是學校發的原始文件，有主管注冊人員的簽字和學校的紅印或鋼印，必須表明你入學的日期，所有科目或課程，學分或學時，以及每一科目的成績（等級）。

真實的複製件，隻有經教務人員或公證員證明確是原件的複製件並親筆簽字後才可被接受。每一經證實的複印件也必須表明授權人員的頭銜，井蓋有印章。
Unless transcripts and diplomas are issued In English by the Institution，the official record their original language must be submitted with an authonzedcomplete，and exact English translation．The translaion document must also bear the seal and the translator’s original signature．
如果學校發的成績單和畢業證書不是英文的，那麼除了提交本國文字的正式文件外，還要有一份被授權單位提供的完整正確的英文翻譯件。翻譯件上也必須有翻譯者的親筆簽字和印章。

大學本科的應屆畢業生可以申請錄取，但校方隻有在收到能表明所有成績的最後成績單和學士學位證書後才能接收。申請人如果不是在美國上的大學，也必須具有相當美國學士的學位。

②關於 GPA（Grade Point Average）。
GPA是全麵反映你在大學期間的學習成績的量化指標，在你的申請過程中，不少表格上都要求填寫GPA。但是由於世界各國的教育體製和評分體係都不盡相同，即使在美國的大學中，各個學校采取的具體辦法也不完全一樣。所以實際上並沒有統一的計算GPA的方法。由於我國目前仍采用百分製評分，下麵就介紹一種轉換體係，供你參考。

有些學校指出，國際學生也可以不進行轉換計算，你可以直接用百分製的平均成績表示。

在填寫時，你也可以將按 4．0 scale計算出的GPA和按百分製計算出的結果同時標出。如 3． 6／4．0（88／100）。
成績單上分數的高低，一般來說，反映了你學習的實力，也是評委們參考的重要依據。若你的某些成績不太好時，可請教授在推薦信中稍加解釋，如所修科目的類別及難易程度，並說明你的學習潛力。不要自己在ESS葉或其他文件中過多解釋，以免評委生疑。
