彩色多普勒超聲診斷甲狀腺癌

[摘要] 目的 探討彩色多普勒超聲對甲狀腺癌的診斷價值,提高甲狀腺癌的診斷準確率。 方法 分析經手術治療並經病理診斷確診的40例甲狀腺癌患者的術前超聲檢查結果,觀察甲狀腺癌的二維聲像圖及彩色多普勒血流特征。 結果 ①本組40例甲狀腺癌中乳頭狀癌占多數,占87.5%(35/40),多數癌腫結節與良性結節並存;②40例甲狀腺癌(44個腫塊)的主要聲像圖特點:腫塊以實質性低回聲為主,占77.27%;腫塊多數邊緣不光整,呈毛刺狀、蟹足樣;腫塊內部多伴鈣化(37/44,84.1%),以微鈣化為主(27/37,73.0%);③腫塊血供豐富,血流檢出率95.45%,且以Ⅲ、Ⅱ級血流為主。結論 甲狀腺癌超聲聲像圖多變,檢查中不僅要抓住甲狀腺癌的主要聲像圖特征,還需多切麵觀察,綜合分析判斷,注重歸納總結,以提高甲狀腺癌的診斷準確率。

[Abstract] Objective To explore the value of color Doppler ultrasound in diagnosis of thyroid carcinoma and improve the accuracy of diagnosis of thyroid carcinoma. Methods Pre-operational ultrasonic characteristics of 40 patients with thyroid carcinoma which were confirmed by pathology were analyzed. To observe two-dimensional ultrasonography and color doppler flow characteristics of the thyroid carcinoma. Results (1)palillary carcinoma accounts for the majority of 40 cases of thrroid carcinoma, 87.5% (35/40),coexist about the majority of tumor nodules and benign nodules; (2)The main sonographic characteristics on 40 cases of thyroid carcinoma(44 masses)were: a.the lump with substantial hypoechoic,accounted for 77.27%. b.the majority of lump with irregular margin,burr shaped,crab pattern; c.calcification appear in the lump (37/44,84.1%), was given priority to with microcalcification(27/37,73.0%); (3)abundant blood flow appear in the lump, detection rate of 95.45%, and given priority to with Ⅲ,Ⅱ blood flow. Conclusion The ultrasonographic of thyroid carcinoma changeable,not only to emphasis the mainly ultrasonographic characteristics of thyroid carcinoma,also need to observe more than one section,comprehensive analysis and judgement,pay attention to the summary, in order to improve the accuracy of diagnostic rate of thyroid carcinoma.

近10年來甲狀腺癌呈上升趨勢[1,2],約占全身惡性腫瘤的1.5%,占甲狀腺全部腫瘤的2.7%~17.0%,且發病年齡集中在40歲左右,嚴重影響人們的生活質量[1]。隨著超聲技術的不斷完善和發展,高頻超聲因其安全、方便、可重複性強等優點成為診斷甲狀腺腫瘤的首選檢查方法[3]。為使甲狀腺癌患者能夠得到早期、及時地診斷和治療,需對甲狀腺癌的二維聲像圖及彩色多普勒血流顯像進一步深入研究,以提高甲狀腺癌的診斷準確率,更好地為臨床提供客觀診斷依據。
